2770 | David Thomas Ballard (55) died suddenly of a cardiac arrest at the Orem Costco on Wednesday, October 27, 2010. David was the second son of Mona Rae Maw and Richard Derrall Ballard, and was born May 17, 1955 in Salt Lake City, Utah. David grew up in Salt Lake City, graduating from Highland High in '73. A high school highlight was the Red Feather Band. He was a gifted, versatile, rock-solid drummer who was sought after by many musicians. David served an LDS mission to Kobe, Japan ('74-'76) and graduated with a BA in Japanese from BYU in '80. At BYU, he finally met and married the love of his life, Patricia Diane Styles, on October 3, 1980 in the Salt Lake Temple. With the "wife of his youth," they had five smart, wonderful children. They enjoyed their thirtieth anniversary just weeks before his death. David served in many positions for The Church of Jesus Christ of Latter-Day-Saints and was currently serving in the High Priest Group Leadership. David began his professional career in international business, working for Interact in Tokyo, Japan (1981); Chiyoda Chemical Engineering Corp ('81-'83); and Mitsubishi Corp in L.A. ('84-'86). After his father's own untimely death, David decided he no longer would work with money--but with, and for people. He received a MA in Social Work at BYU in '88. After earning his LCSW, he worked for various organizations, including Utah State Prison, where he helped develop the mental health and woman's substance abuse programs. After ten years at the prison, he developed a thriving full-time private practice, working with all ages and aspects of mental health. He received his twenty-year recognition from IHC for serving as an on-call, crisis social worker. He was a respected clinician and beloved therapist. David was a World War II enthusiast and voracious reader of all things non-fiction. He had an insatiable curiosity spanning all genres and eras. He participated in two archeological digs in Israel; scuba dived in all the usual places; had a shark encounter in the coral reefs of Ras Mohammed; flew twice in a World War II B-17, once each in a B-24, a B-25, and a Skyraider, and helped in the restoration a B-17 in Kissimmee, Florida. (He will be disappointed that the exact nose versions of these planes are not identified.) He experienced many tender mercies from the Lord. He followed the voice of the spirit, which led him to many productive experiences, friendships, and adventures. David loved his children, the doctrine of the LDS Church, and helping people solve their problems. He challenged his children to work hard, be strong, and be true. David is survived by his wife, Diane; children, Jessica Rose, Justin David (Christina), Mackenzie Rae, Walker Thomas, and Mckay Wesley; his mother, Mona Rae Maw Ballard Cole; and brother, Marc C. (Julie) Ballard. He was preceded in death by his father, R. Derrall Ballard, and his brother John L. Ballard. Funeral Services will be held on Saturday, November 6, 2010 at 10:00 a.m. in the American Fork Thirty-Seventh Ward Chapel, 396 North 1100 East, American Fork, Utah. Please celebrate David's life at a viewing on Friday evening from 6:00 to 8:00 p.m. also at the Church and an additional viewing at the Church on Saturday morning from 9:00 to 9:45 a.m. prior to the services. 2789 | Dean 'Casey' Russell Mortimer, 89, of Blue River died Monday, March 13, 2017 at Boscobel Care and Rehab Center. He was born on October 23, 1927 in Valton, Wisconsin the son of Dolph and Millie (Stowe) Mortimer. Dean attended Valton Grade School and Wonewoc High School. Upon graduation, he attended Wisconsin State University in Platteville for two years, where he majored in agriculture. From 1951-53, Dean served in the US Army in Korea and was a member of the American Legion and the VFW. After Dean's honorable discharge, in 1954 he completed his Bachelors of Science Degree in Agriculture from Platteville. Dean began his teaching career in Blue River. It was there he met Connie Powers and the couple was married on June 14, 1956. Dean and Connie moved many places but it was in Greeley, Colorado where he received his Masters Degree in Guidance and Counseling. The last move was to Kankakee, Illinois where he attended the University of Illinois for advanced counseling. It was in Kankakee he completed 37 years of teaching. The couple finally retired to Blue River in 1991. Dean was an active member of the Blue River United Methodist Church. Dean loved to fish and hunting, especially when he lived out west. He and Connie were fortunate to go on many trips after retiring. Dean is survived by his wife of 60 years Connie of Blue River; three daughters: Denise Holmes of Bloomington, IL, Marcia (Dr. Barry) Erdman of Menasha, WI, Cheryl (Tom) Carlin of Boscobel, WI; six grandchildren; two great-grandchildren; sister: Muriel (Bliss) Blakeslee of Ironton; brother: Duane 'Dick' (Beth) Mortimer of Columbus; many nieces, nephews, other relatives and friends. Dean was preceded in death by his parents Dolph and Millie, two brothers Ronald 'Mike', Walter 'Pat.' Funeral services will be held on Sunday, March 19, 2017 at the Blue River United Methodist Church at 12:00 PM, with Pastor Jeanne Jones officiating. Burial with military graveside rites will follow in the Blue River Cemetery. A visitation will be held on Sunday, March 19, 2017 at the Blue River United Methodist Church from 10:30 AM until the time of the service at 12:00 PM. 2791 | Dear Johan I did notice the Maw family references too - you may not be aware that GEORGE MAW (1832-1912) also had a heather named after him (Erica ciliaris 'Mawiana') as well as quite a few other plants [he was an enthusiaistic amateru botanist - I see him described as a taxonomist on your tree which is certainly not his profession, even if it was his avocation in his spare time. But he was also an artist, praised by John Ruskin, no less. Looks as if there is a streak of botany and horticulture in your family tree! If you wanted I could scan his portrait and his heather for you. On the Doncasters, yes you could attach the email. This, by the way, is Samuel's signature (I don't have his portrait) from a copy of the Backhouses' heather catalogue (1911) in which the heather was named after his wife. 2792 | Dear Johan and Judy, Sorry for the delay in replying. Please find attached a scan of two pages from the new Chelsom lighting catalogue covering the story relating to John Parker Maw. Chelsom are one of the UK's leading restorers of antique lighting and as you will see from the story we have restored the main chandelier from St Johns Cathedral, Antigua. During restoration we found inside the note handwritten by John Parker Maw on February 27«sup»th«/sup» 1886 when he cleaned the chandelier. He arrived in Antigua April 26«sup»th«/sup» 1883 presumably from his place of birth Thornton le Dale near Pickering Yorkshire England. We think you have his date of birth and he appears on two censuses for that town. I believe he left Antigua shortly after this time and appears on a census in New York. Also attached is a copy email from the Project Manager in Antigua with some information about the firm who John Parker Maw was working for. The original note handwritten by him has been restored and framed and will go back to the cathedral with the chandelier. There is some delay on finishing the restoration of the building itself so we are holding the chandelier here in the UK until it is ready. 2793 | Dear Johan and Judy, Sorry for the delay in replying. Please find attached a scan of two pages from the new Chelsom lighting catalogue covering the story relating to John Parker Maw. Chelsom are one of the UK's leading restorers of antique lighting and as you will see from the story we have restored the main chandelier from St Johns Cathedral, Antigua. During restoration we found inside the note handwritten by John Parker Maw on February 27«sup»th«/sup» 1886 when he cleaned the chandelier. He arrived in Antigua April 26«sup»th«/sup» 1883 presumably from his place of birth Thornton le Dale near Pickering Yorkshire England. We think you have his date of birth and he appears on two censuses for that town. I believe he left Antigua shortly after this time and appears on a census in New York. Also attached is a copy email from the Project Manager in Antigua with some information about the firm who John Parker Maw was working for. The original note handwritten by him has been restored and framed and will go back to the cathedral with the chandelier. There is some delay on finishing the restoration of the building itself so we are holding the chandelier here in the UK until it is ready. 2794 | Dear Johan, I can now confirm the details of my great uncle, William Ross Griffiths' death, having got a copy of the death certificate. He died on 28 February 1938 at 31 Fore Street, St Ives, Cornwall. His son-in-law, Herbert Wedmore Martin was present at his death. His wife, Emma, died at the same address on 21 March 1942. This was a few doors away from 39 Fore Street, where the Martins carried on a drapery business. In 1911 Herbert was living there with his parents (from whom he inherited the business). 2795 | Dear Johan, I've done a bit more searching for information about the death of William Ross Griffiths. Unfortunately I can find no record of his death either as it was registered, or in the National Probate Records under his full names. So the problem is compounded by the large number of William Griffithses. For the latter part of his working life he was working for the Great Western Railway in Birmingham. I had assumed that he died there and that his widow, Emma, moved down to St Ives after his death to live with her daughter, Doris, who was, by then, living in St Ives with her husband, Herbert Wedmore Martin. You may, of course, be right in thinking that the William R Griffiths, whose death was registered in Penzance in 1938 was my great-uncle. So I will take a pot-shot and get this person's death certificate, which should include his occupation, even if it does not state his second forename. I'll let you know the result. If this fails, I'll have a go at the most likely Birmingham death. I see that you also have his brother, Frederick Francis on the Maw website. I have more details about him if you want to add them. 2796 | Dear Judy Sorry to be a while answering your message, I'm engrossed in parish records relating to my ancestors at the moment. My husband is the connection, his Aunt Dora Edith Orchard married David Edward Maw. I have a copy of their marriage certificate. They married on 01/01/1924 at The Register Office Rotherham. David Edward Maw was 19 a bachelor - fireman at Steelworks and living at 18 Temple Street, Templeborough. His father was Edward Maw a Bricklayer/Labourer. Dora Edith Orchard was 20 a spinster living at 19 Cross Street, her father William Orchard a coal miner. I have to say that Gary never knew his Aunt Dora, there was a big gap between the eldest Orchard sibling and the youngest (Gary's Dad was the youngest) Gary knew that his father was one of quite a few children but didn't know the details properly until I started researching his family tree. David and Dora Maw had a son David A Maw born c1924 in Rotherham. I don't have any other information on the Maw family I'm afraid. 2797 | Dear Judy, I believe I had correspondence with Johan quite some time ago about the Maw family. I find I have a date that could be useful to you. In an old "Birthday Book" that was the property of an old (and now deceased) great or great-great Aunt of mine I have an entry that say that Mrs Maw's birthday was on 6 Jun 1820. Given that Frances Maw (née Holtham) was the only Mrs Maw in the family and that there were several other birthdays in the book relating to the period in Leicester, I'd be pretty sure this is the correct date for Frances.
